This page is for recording incidents in the workplace that require the attention of management. These can relate to safety, wellbeing or cultural problems within the workforce, or anything else you think needs to be discussed, recorded or reported.

Your report will be immediately sent to management, and will be responded to at the earliest possible opportunity.

This report will be kept in confidence and further steps assessed as per the action required.
